Random Surf Protection Screen

Has anyone else had issues with 8.2 giving random surf protection screens every now and then. Its getting annoying having to close my browser each time to sort the issue. We are using AD SSO authentication with the HTTP proxy. Normally the log would show my user name in the log but when this happens the username is blank. Here is one of the blocked entries:

This also shows up in the client authentication log:
