Connection to server timed out!

I have this configuration:

Host: Windows Server 2003 R2 SP2 Standard x64 with 2 NICs
VM: UTM 9.3 (VirtualBox)

Interfaces:
Internal : Ethernet on eth0 [192.168.1.254/24] MTU 1500 
External : DSL PPPoE on eth1 [197.***.***.202/32] MTU 1492 DEFAULT GW 197.***.***.1

DNS:
Allowed Networks: Internal
Forwarders: Google DNS (8.8.8.8, 8.8.4.4); Use forwarders assigned by ISP: Unchecked
Request Routing : My Internal Domain --> My Internal DC

DHCP:
Internal [Range 192.168.1.120 through 192.168.1.150]
DNS 1: 192.168.1.254 DNS 2: 8.8.8.8 Default Gateway: 192.168.1.254 WINS: 0.0.0.0

Firewall:
Internal --> DNS, Email Messaging, File Transfer, Terminal Applications, Web Surfing --> Any

Masquerading:
Internal (Network) --> External

Web Filtering:
Allowed Networks: Internal
Transparent Mode
URL filtering only 
Policies Allow ALL

Email:
SMTP disabled
POP3 disabled

From time to time I got this message:
An error occurred while handling your request
Connection to server timed out 

How should I fix it?

Another problem is that sometimes I cannot send emails using Outlook unless I add a Firewall rule [Any --> Any --> Any]

How to fix it?
