Protect > Applications > Application Filter > Block filter avoidance apps ... need default config

After a bit too much mucking about, I want to reset the application filter "Block filter avoidance apps" to the factory default.

Navigate to Protect > Applications > Application Filter > Block filter avoidance apps. You'll see three rules. For each of the three rules, please transcribe exactly what appears under the column "Application Filter Criteria".
