After update to v16 only one time login is possible in user portal

did you enable OTP feature inside the XG?

Can you share User Portal Logs?

Do the following:

• Connect to XG CLI
• Press 5 and then 3

• Enable debug for authentication service :  run the command "service access_server:debug -ds nosync" (to enable debug for authentication daemon)

• tail -f /log/access_server.log  (and logon on user portal )

• Share the access_server.log file

•  

  Run again the command "service access_server:debug -ds nosync" ( to disable debug)

Launch command without quotas.

from the logs, OTP is enabled. Make sure to disable all the OPT toggle switches and try again.

It is not sufficient to switch off the main switch only. You must disable all options for OTP!!! I think it is a bug, because you expect all ist disabled when you switch off the main switch!
