Port forward in and out 44158 Helium hotspot

I’m having difficulty with my new Helium hotspot communicating in and out on 44258.

heres troubleshooting from Helium when the hotspot is in Relay.  “We recommend enabling TCP Ports 44158 in both directions in your router. Check your Router's manufacturing instructions on how to open ports 44158.

I have a NAT, DNAT with loop back and reflexive, but only getting out, not inbound.

Any help?
