Two Sophos icons/shields showing in toolbar?

And there are 2 UI controller proceses running.

I will check that the next time it happens. Watch this space for a response from me when I see it occur again...

same here, also the system gets awfully slow and even hangs....

Have you guys looked at the other threads on here about two icons to see if the fixes work for you?

Andrew, the only thread I saw was to uncheck Sophos from startup items. Mine was already unchecked so it isn't that.

Are there any other recommendations for stuff to try?

The problem just happened again, and yes, I can confirm that when this happens, there are 2 x SophosUIServer processes running.

Does this help?  Any ideas what the problem might be... or if this is a bug?

For me the same. Two icons and two processes running. When I stop one of the 2 processes, one icon disappears.

I, too, had the two Sophos icons in the menu bar as well after my Mountain Lion upgrade.  I noticed that someone mentioned that they removed the checkmark in the startup items list.  That checkmark just means that the item is hidden when it automatically starts up.  You must remove the Sophos line item from the startup items list all together.  Once you do, then reboot your Mac.  After startup, you should see only one Sophos icon in the menu bar.

Yes, the UI is a bit misleading there (I've fallen for this a few times myself) -- the checkmark is for selecting whether the item launches in the foreground or the background.  Clicking the "-" to remove the item is what's needed, as mentioned previously in this and other threads.
