Issues with HTML5 VPN Portal

Hello smart people:

I have a UTM 220 running 9.209-8. We are using McCafee cloud proxy service to filter and control web access for our employees in the field. I have a short-term notice need to get the laptops able to the HTML5 VPN portal, but I am having some difficulties with the proxy server. If I disable the proxy, everything works fine (IE 11 on Win7).

With the proxy enabled, but configured to bypass the proxy for the server URL, I get the following symptoms:
1) Login screen layout is shifted (see attached image)
2) When connecting to the VPN Connection, I get the message "A modern web browser is required to use this feature. This includes (but not limited to) Internet Explorer 10 or later, Firefox 6.0 or later and Google Chrome".

Does anyone have suggestions on what else I can tweak to make this work? At this point I have disabled all filtering rules at the the McAfee proxy although traffic still routes through it for non-bypassed URLs. Is there any additional sites I should have IE bypass the proxy for? I did a report from McAfee's site and tried to include all the sites it saw my laptop as accessing but things are still not working correctly.
