SG 135 dropping initial packets

One pair of our SG135s is exhibiting a strange behavior - the initial packet seems to drop for outbound traffic.  For example:

I ping www.google.com from behind the 135s - first ping reports destination unreachable, all subsequent pings go through fine.  It's not a DNS issue as it happens if I ping by IP as well.

However, the issue doesn't happen when I ping from the 135s Ping tool in the Web UI, it only happens from machines behind the firewall.  For example:

- I ping 8.8.8.8 from Machine A behind the 135s - first ping fails, all subsequent pings work.

- I ping 8.8.8.8 from the 135 itself - all pings successful.

We have two balanced ISPs at this site with several multipath rules, however this happens even when one or the other of the ISPs is disabled as well, so it's not some failed multipath / routing issue as far as I can tell.

We're running 9.355-1 here and at other sites, and this is the only site exhibiting this behavior.  Any suggestions are welcome, and I can provide any other relevant info.
