Voip helper not working with h264

I'm trying to use the network protection -> VoIP helper.
It works fine as long as we only make voice calls.

When I try and place a video call (h.264), just get a timeout.  Nothing at all shows up in the firewall log.

Turn off VoIP helper, and everything works again.
I can't figure out how the VoIP helper operates, and where it is logged.
