How secure is this?

Eventually I am going to stick the mail server on its own network card out of the ASL but at the moment the mail server is on the internal network, I have setup NAT so that any external addresses on port 110 go to the mail server on port 110.  Works no worries...but...how secure or unsecure is this scenario?

    Depends on the (security) quality of the mail server software you are running. When you have a port that's open, that's like exposing a lock for a lockpicker, and he knows he will not be subject to scrutiny while he's trying to pick it. He will have time to play with it. It's unlikely you will be watching the activity closely enough to see when somebody is getting cute.

    Put it on your To Do list to put it on its own NIC; if your domain is not a high profile one, you will probably be safe for a week or two; if you are guarding very valuable info, don't take those chances...
