QoS Question

I'm confused.  Can someone give me a little better description on how (or if) I can utilize QoS in a NAT'd environment to guarantee service level for an internal IP address?  Based on the online help, it doesn't sound like doing QoS would work.  I have an internal VoIP device that I have given a reserved address to.  I would like to assure that this device will always get 200K bandwidth.  Also, would like to make outgoing FTP requests/traffic get a lower priority than everything else.
