SMTP Proxy sends duplicate e-mails

Recipients of mails sent out of my site by the ASL SMTP proxy are reporting occasional multiple copies of some mails.  Some e-mails, particularly ones with attachments it seems, are sent anything from 2 to 9 times with the same message ID.

The live log is also showing strange behaviour:

Also, different e-mails sent long times apart have the same filename, for example:

Mails sent are received successfully, but I think the re-use of filenames is something I never noticed before, and may be related to the duplicate messages.

A few weeks back I deleted some mails from the queue using the rm |find command, and also had to delete some files from the log because I received:

I think this could be a bug, and other users on this BBS have reported similar behaviour.
Otherwise I've been highly impressed with this comprehensive and thorough firewall.
