Some emails are going missing

We had some complaints starting 2 weeks ago from one person, so I thought it was their problem. But now it is showing up from some other people.

From what I can tell, they are not getting bounce messages, but we are seeing no sign of them either.

They are not showing up in SMTP Log. I even turned off "Reject Malware during SMTP transaction" and Blackhole confirmed virus. What is going on? 

I upgraded to 8.203 after we started getting the first problem. I have checked blacklists for the problem mail server, including our own.

Any other ideas? We are getting most of our email, which is why this is a puzzler.

ADDED: This is scary. On a whim I added the first problem email domain to the exceptions list and the test email came right through. 

I thought the SMTP log was there to record every single action of the mail scanner. I can't fix a problem that I can't see. What happened?
