Error during installation

When I try to do a fresh installation of 5.0, it fails each time at the formatting the log partition step.  If I don't do a cold boot, it will then not see the hard drive anymore.  Has anyone seen this before?

    IME if an hdd is not seen upon a warm reboot usually the hard drive is dying.   Other possible causes are a bad ide(or scsi cable) or a bad disk controller(for ide this means a bad motherbaord..for scsi either a bad card or bad motherboard depending on hwere the controller is).  Try moving the drive to another controller(in the case of ide).  Also try another data cable.  lastly try another hard disk.
