Dropped Packets?

When I ping -t the next hope of my firewall I get packet loss.  about 60%.  Anyone have any ideas?

    Bad Ethernet cable?  Bad Ethernet NIC?  Mismatched MTUs?  NIC and switch/router not agreeing on duplexing and or speed?
