Can't login via web interface or loginuser

Hello all.

For some reason, my authentication for the web administration or loginuser are not
recognized or authorized.

Just a few days ago, I had to manually reset the web admin password by using the
loginuser and system_password_reset combo. Now, it's happening again. Only this
time, the loginuser account is also locked out.

Has anyone ran into this problem and know the fix?

    Hard reboot of the server was my last choice. Fortunately, I was able to access the web
    admin after that. The loginuser account was still locked out but was easily reset from the
    web admin.
