Issues with wireless clients getting dhcp address

I am currently having issues with my wireless clients receiving a dhcp lease. They will sometimes work, and other times not. Wired clients have no issue.

Setup

Sophos UTM running firmware 9.509-3

WAP: Ubiquiti AP Lite running firmware 3.9.27.8537 (fairly current)

Switch: Cisco ESW-540-24P

My end goal is to have the network setup as follows:

Vlan1 - Primary wired clients (Amazon Fire TV, Xbox, network devices, etc). This vlan will also be the least restrictive and will have me and my wifes devices on it

Vlan60 - Kids wifi. Web filtered vlan for the kids.

Vlan61 - Guest wifi. Lightly filtered for guests with wifi psk I can destroy at will.

I have removed and/or disabled a lot of my extra config while troubleshooting these wifi dhcp issues.

Things I have tried

- I used to have a Meraki WAP that had the same issues. Used internal dhcp to Meraki to resolve until I took time to troubleshoot. Free license ran out, so put in Ubiquiti.

- Tried multiple ports on switch. Defaulting switch port config. Verifying vlan tagging.

- Verified setup in WAP is correct. No vlan tag on default ssid. Others were tagged properly with 60, 61.

- Defaulted switch and WAP and reconfigured.

Right now I am at a loss as to what could be causing the issues, as it will work on occasion but not all times.

Anyone have any ideas/thoughts as to what could be going on?
