[7.460][FEATURE][FIXED] new DHCP mapping missing Comment field

When creating a new DHCP mapping from the lease table, there is nowhere to add a comment.
(There is a comment field on the static mappings page.)

Adding a comment field would be useful. Then I could remember why I assigned a particular IP to a computer/user/Xbox360/fridge
