OTP Issue

We have recently setup AD and added a number of users into our Sophos XG. I know would like to configure OTP - this was straight forward but when a user first sets up their OTP and Sophos authenticator they end up being asked to do the same thing over and over. This prevent the users from using the IPSEC VPN, its no longer recognises their account and I'm forced to switch OTP off to allow anyone to login. 

I've looked at a number of similar issues on here but i've been unable to find a solution. 

Is there any way around this?

    Thank you for contacting the Sophos Community.

    Can you SSH into the XG and press 5>3 to be in the Advanced Shell and see what does the following log says after you put it in debug and try enabling again.:

    # service access_server:debug -ds nosync

    /log/access_server.log
