submit link in user notification when website is filtered is not working

i created a firewall rule for web filtering, used the default workplace policy 

i am testing it before applying to all, tried to open bevmo.com i got the nothification :

 

This is a message from the IT Department. 

The web site you are trying to access: 
http://www.bevmo.com/
is listed as a site within the category Alcohol & Tobacco

Current Internet Access Configuration for you does not allow visiting sites within this category at this time.

If the website has been erroneously blocked, please submit it for re-evaluation.

 

when i click on submit nothing happne while it should send submit request to the admin IT, when i copy the link and paste it it will show bevmo.com

 

please help

    Did you migrate from UTM?

     

    Go to Web, Notification Settings (might be behind …)

    Turn off the custom messages.  That will get you to the defaults.

    If you want the custom messages (assumed migrated), we would need to figure out what is wrong with the custom html.

    The default is
