Issue with DHCP IP pool assignment

I have created separate local networks on separate interfaces of firewall as:

The DHCP pool is created as:

But the issue with systems on 192.168.3.0 network is some systems on random basis are getting IP from 192.168.4.0 network, even though they are connected 3.0 network.

Each network is separated from firewall to each department via CISCO switch.

No vlan, no bridging done then why ip is coming from different pool??

Although those systems are able to access everything easily but still this issue is causing improper IP assignment whether LAN system or Wi-fi laptops connected via Cisco AP.
