[Update] 22/11:
Issue identified and (as of 11:30 am EDT) update applied to reduce the potential for this happening (in the interim of a full solution). Please re-attempt to delete an item if it had failed previously.

[Update] 12/4:
No new incidences of failure to delete have been seen following the 11/22 interim fix. Long term fix is scheduled for December 7th.
